Each player mills ten cards. For each player, choose a creature or planeswalker card in that player's graveyard. Put those cards onto the battlefield under your control. Then each creature you control becomes a Phyrexian in addition to its other types.

Why is this card good?

Breach the Multiverse is a top-end reanimation payoff that steals from everyone at once. At seven mana you mill all four players ten cards, then grab the best creature or planeswalker from each graveyard—often pulling 3-4 bombs you didn't even have to set up yourself. The big draw is symmetry working in your favor: you're not just reanimating your own dudes, you're plundering opponents' fatties and walkers, which is both a swing and a tempo blowout. It plays great in reanimator and graveyard value decks (Sheoldred, Tergrid, Chainer) and any black midrange pile that wants a powerful finisher. The Phyrexian rider is mostly irrelevant unless you have tribal synergies. Downsides: it's a do-nothing without targets, so it whiffs early or against graveyard hate/exile-based removal. Don't run it in low-curve aggro or stax shells that want to stay lean. It's also a magnet—people will counter or kill you for the value. Best when your meta has fat graveyards to raid.
